The reason the ICT adapters come loose during play for some is because the lock washer is too big in diameter and smashes into the bore.
The lock washer must fit freely in the hossle bore or it looses it locking function.
To fix it, I just lightly ground the O.D. of the washer on a bench grinder until it fits freely in the bore. Then tighten the adapter. Re-loosen and check that the washer is not tight in the bottom. If it is, remove and grind some more of it.
This is a great design but the washers should have been smaller.

After using the driver for the entire holiday weekend I've learned the washer diameter is not the only problem. Where the ends of the washer comes together when compressed, kind of fused the ends together, not allowing the washer to lock the screw. I had to retighten every four or five shots. I filed the ends to allow about 1/16" clearance when compressed and that seemed to help. I hit it 14 times on Sunday and it is still tight. The washer could be made of a little better grade of material. It seems a little soft for the function it is suppose to acheive.
