Yes it has a TPU or thermoplastic urethane insert.

My questions:
1. Of what material is this putter head made?
2. Is it face-balanced?
3. Can it be bent for loft and if so, does Hireko offer it?

Norman,

The body is made of zinc and cannot be bent for lie or loft. This model has a toe hang.

In the over head photo, it appears the center alignment line does not match up with the white dot, which appears to be aligned with the center of the crosshair on the face. Is that true? Can you pick up the ball with the back of the putter head?

The alignment dot is centered. The angled that the images was shot makes it appear as it wasn't. Yes, you could scoop a ball up with the back of the putter.
